Triage Specialist

Summary of the Role
The Triage Specialist reports directly to the Senior Director of Triage and Guest Ministry and serves as an initial point of contact for shelter guests. Responsibilities include completion of the Triage questionnaire, assessing and identifying barriers that brought the guest into the shelter, encouraging guests to consider shelter services tracks, and/or connecting them with resources in our community. Triage Specialist is responsible to provide and record all related documentation. The Triage Specialist provides communication and a warm introduction to the case manager. This position provides spiritual support, guidance, and encouragement for all guests.

Job Plan
- Welcome new guests entering the shelter, complete Triage and HMIS ROI, document processes completed, and conversation.
- Determine if shelter guests have completed a VI SPDAT and if not, assist in scheduling for completion either through HAP or onsite. Demonstrate proficiency in understanding and administering VI SPDAT. Ensure that they are completed within the required timeframe, as appropriate.
- Provide solutions-based case management through identifying resources either within MTM or in the community to support guests successfully moving out of homelessness.
- Maintain open communication with case managers on referrals made. Assist with preparing the guests for entry into selected services.
- Demonstrate proficiency in VCM, Resident Tracker, and HMIS. Ensure accuracy and timeliness of all entries.
- Assist with all Intake duties

Qualifications
- Bachelor's Degree or equivalent years of experience in a related position
- Skilled in trauma-informed communication and de-escalation
- Minimum three years working with an underserved population
- Proficient with Microsoft Office Suite, Resident Tracker
- Strong base of knowledge for HMIS
- Proficient with information management software platforms, or able to learn.
- Ability to work independently and with little supervision
- Ability to communicate our vision professionally and passionately
- Highly organized, detail-oriented, able to manage multiple projects
