Extended Care Program Case Manager

As the Case Manager (Extended Care Program) you will support the ministries of Women and Children Ministries (WCM) by providing case management, care, counseling, spiritual leadership and records management for women in the Extended Care Program (ECP) and Oaks Addiction Recovery Program.

RESPONSIBILITIES

*Process and conduct intake assessments for Oaks and Extended Care Program.
*Manage a caseload of individual clients in the addiction recovery program and Extended Care Program.
*Meet with clients weekly to discuss personal, medical, legal, or other issues to determine their status and to ensure they are meeting their established goals. Maintain all electronic and paper client files.
*Help in maintaining the safety and cleanliness of the WCM facility.
*Provide crisis intervention, chemical dependency case management, and referral help.
*Communicate with all staff through the Client Tracker (CT11).
*Help coordinate and facilitate support groups, Bible studies, addiction recovery curriculum, and classes.
*Attend various Mission functions, meetings, and training sessions, as required.
*Help maintain a positive, pleasant, and Christian atmosphere at HP.
*Maintain program policies and procedures for the ECP program.
*Meet weekly with appropriate WCM staff to review clients goals, medication needs, and progress.
*Meet weekly with Supervisor to discuss the status of women assigned to the ECP program and any pertinent program information.
*Maintain order, safety, and peace in the ECP wing through appropriate involvement in the conflict, resolution, and crisis intervention.
*Administer observed urine collections for guests.
*Perform welfare checks in guest residences and restrooms.
*Due to the sensitive nature of these essential job duties, we are only able to consider females for this position.
*Maintain partnerships with supporting medical centers and physicians.
*Work with the Volunteer Department to identify volunteer support for the program.
*Attend and participate in all Mission, Division, and Department meetings and training, as required.
*Any other job-related duties as assigned by supervisor.

QUALIFICATIONS & EXPERIENCE

*BA/BS in Social Service or other behavioral science is preferred.
*Certified Nursing Assistant or training in the medical field is preferred.
*One year of case management experience is preferred.
*Nursing or medical field experience preferred.

WORK ENVIRONMENT

While performing the duties of this job, the employee is frequently required to sit for long periods of time in a temperature-controlled environment. Requires frequent use of computers, keyboarding, and talking on the telephone. Occasionally will stand, balance, twist, bend, stoop, kneel and reach with hands/arms. Regularly lifts up to 20 pounds and walks using stairways to climb to multiple floors within the facility. Specific vision abilities required for this job include close vision, color vision, distance vision, depth perception, and the ability to adjust focus.
