Navigation Center Case Manager

Under the supervision of the Director of Emergency Services and the Associate Director of Navigation Center Operations, the Navigation Center Case Manager will work intimately with service providers, community partners, and volunteers to create pathways to health and recovery for people seeking to move out of a houseless reality and into permanent and supportive housing. In this capacity, the Case Manager will work individually with clients to learn their story and formulate a specific plan to help them move into a healthier and more stable lifestyle. Additionally, the Case Manager will assist the Associate Director in creating and maintaining a safe, supportive, and healing environment for people to address physical, emotional, and mental needs. The Case Manager will perform his or her duties and responsibilities to care for the needs of our Navigation Center guests, to enhance fulfillment of the Navigation Center’s mission, to collaborate well with all community partners, and to provide a safe and healing environment to all present at the Navigation Center.
